Kassidy Hardee PTA
Kassidy Hardee earned her Associate in Applied Science – Physical Therapist Assistant from Murray State College. Prior to that, Kassidy obtained her Bachelor of Science in Dietetics and Institutional Administration from Texas Woman’s University.
Kassidy specializes in orthopedics.
In her free time, Kassidy enjoys spending time with her husband and four boys.

Physical TherapistNick was born in Columbus, Ohio, and later moved to Austin, TX where he spent most of his childhood. He played several sports growing, primarily focusing on baseball which he played at the college level at the University of Mary Hardin-Baylor in Belton, Texas. After an injury ended his sports career, Nick obtained his Bachelor’s in Science from UMHB.
He moved to OK and received his Doctorate in Physical Therapy from Oklahoma City University. He is currently a member of the American Physical Therapy Association (APTA) and is pursuing residency and specialization including an interest in a variety of orthopedic and sports injuries.When Nick is not in the clinic, he enjoys watching and playing sports, especially rooting on the Ohio State Buckeyes football team, being active, eating at local restaurants and spending time with his wife and dog Maple.

Rachel Hazewinkel PT, MPT, CDNP
Rachel has been a practicing therapist since 2009 with an emphasis on outpatient orthopedics and sports related injuries, in particular shoulders and knees. In 2016, she received her certification in Dry Needling.

Nathan was born and raised in Lone Grove, Oklahoma. After graduating high school in 2013, Nathan attended college at Southwestern Oklahoma State University. At SWOSU, he studied kinesiology and instrumental music, and was involved with Phi Mu Alpha Sinfonia music fraternity. In 2016, Nathan transferred to Murray State College in Tishomingo, Oklahoma, to attend the Physical Therapist Assistant program. In 2018, Nathan graduated from MSC with an Associate’s degree in Applied Sciences- PTA, with Summa Cum Laude honors.

Physical TherapistDustin was born and raised in Oklahoma City, Oklahoma. He graduated Summa Cum Laude from Classen School of Advanced Studies. He graduated from University of Oklahoma with a Bachelor of Science in Health and Exercise Science. Throughout his college years, he worked as a physical therapy technician at various locations including Physical Therapy Central Midtown where he affirmed his passion to become a physical therapist. He went on to obtain his Doctorate in Physical Therapy from Baylor University.
During PT school, Dustin completed a 6 month clinical rotation at Physical Therapy Central Bethany which helped provide a great foundation in manual therapy, orthopedic and sports physical therapy. His interests include manual therapy, orthopedic, trigger point dry needling, and sports injury prevention and rehab. He is currently working towards obtaining his Orthopedic Board-Certified Specialist (OCS) certification.
Dustin enjoys spending time with family, friends, and his dog Ollie. He also enjoys going on hikes, lifting weights, and playing various sports. He is a huge Oklahoma City Thunder and OU football fan. Dustin is looking forward to meeting you and providing quality care to help you achieve your goals!

Cammie Hodges PT, DPT
Cammie is originally from Amarillo, Texas, where she grew up with her parents and younger brother. She attended Tascosa High School and graduated as part of the National Honor Society and top 10% of her graduating class in 2007. She was a competitive gymnast, cheerleader, and volleyball player, and a member of the high school’s competitive and show choirs while in Texas. After high school, she attended College at Amarillo College, where she completed Associate degrees in General Studies and Sports and Exercise Science in 2009. She then attended West Texas A & M University, where she completed her Bachelor’s degree in Exercise Science graduating Magna Cum Laude in 2013. While completing her undergraduate work, she worked as a physical therapy technician at BSA Hospital in acute care, wound care, and inpatient rehab settings, at Zach Thomas Physical Therapy, and as an assistant coach for cheerleading and tumbling. After graduating in 2013, she attended Texas Tech University Health Sciences Center, School of Allied Health Professionals and completed her Doctor of Physical Therapy degree in May of 2017.
Cammie first found her passion for physical therapy while coaching and working with a pediatrics special needs tumbling class, and has since developed passions for working in underserved communities and pelvic floor rehabilitation. Cammie is seeking advanced certifications in Orthopaedic Physical Therapy and Pelvic Floor Rehabilitation. She is excited to start this new journey in her life in Western Oklahoma helping people get “Back to Work, Back to Play, Back to Life.”
In her free time, she enjoys hiking, reading, spending time with her family, and training and playing with her dogs, Merlynn and Huckleberry.
Kristie Hodges PTA
Kristie Hodges PTA
Born and raised in Moore, Oklahoma, Kristie went to Moore High School, where she was a member of the MHS swim team and graduated in 2004. After high school, she continued on with competitive swimming with a scholarship to Drury University (DU) in Springfield, MO. Kristie was a part of the NCAA Division II National Championship team specializing in all Breastroke events.
Kristie graduated from Drury University in 2008 with a BA in Exercise Physiology with a minor in Global Studies. Following graduation, she moved back to Moore, OK and was accepted in the OCCC PTA program, graduating in 2011. She has worked for the same employer for over 9 years now with special emphasis on shoulder rehab and sports medicine.
In her free time, Kristie loves spending time with her husband and their awesome Golden Retriever, Luna. Their favorite activities include any thing involving tennis balls, lots of cuddles and going on long walks.

Jeff is proud to be raised and living in Choctaw, OK, graduating in 1993 from Choctaw High School. He received a Bachelor of Science in Health and Sport Science and a Minor Degree in Spanish Language from the University of Oklahoma in 1999. In 2001, he earned his Physical Therapist Assistant license and pursued experience in spinal dysfunction, sports injuries, and orthopedic rehabilitation areas of physical therapy.
Jeff continued his education by earning his Master of Science in Physical Therapy from in 2005. He was recognized for the 2005 Texas Woman’s University Outstanding Service Award for exceptional community, career, and personal achievements. Upon graduation, he rejoined his physical therapy family with Bridgit Finley, PT, DPT, M.Ed, OCS, of Physical Therapy Central of Norman.
In 2007, his career aspirations of returning to Choctaw to serve his community were reached when he proudly opened Physical Therapy Central of Choctaw. Jeff feels he is blessed every day by the path he has chosen and the opportunities that have been provided to him. He is grateful to all those who have helped him and to his community that have received him so well.
In 2010, he became a Board Certified Orthopedic Clinical Specialist, demonstrating advance skill and education in the area of Orthopedic Physical Therapy. He enjoys involvement in the local community and Choctaw Chamber of Commerce.
Jeff is married to his wonderful wife, Melisa, and has four children, Kalen, 9, Sarah, 7, Kai, 4, and Kinsley, 2.

Austin was born and raised in Jefferson City, Missouri. He graduated from Jefferson City High School top five percent in his class, where he was an all-district football player. After college football at Central Methodist University, he transferred to Missouri State University where he graduated with a Bachelor of Health and Exercise Science. He earned his Doctor of Physical Therapy degree from Missouri State University. Austin is currently an active member of APTA.
His areas of interest include manual therapy, prevention and rehabilitation of sports injuries, post-operative rehabilitation, and spinal dysfunctions.
Austin enjoys playing and watching sports, spending time with family and friends, staying active, and staying up to date with shows and movies.

Shalyn was born and raised in Oklahoma City, Oklahoma. In 2009, she graduated from Putnam City North High school where she lettered three years on the swim team. She then went on to graduate from Southern Nazarene University with honors in 2013 with a Bachelor of Science in Kinesiology. She earned her Doctor of Physical Therapy Degree from Southwest Baptist University in May of 2016.
Shalyn completed clinical rotations in Missouri and Oklahoma where she was able to gain experience in orthopedic, neurologic, and pediatric settings. Shalyn has been an active member of the APTA and OPTA since 2016, and is a member of the orthopedic section. She plans to become board certified in orthopedic physical therapy, following manual therapy certification in the future.
Shalyn enjoys spending time with her family, friends, and husband, traveling, being outdoors, watching sports, and shopping.

Clinic DirectorBurgundy is a graduate of Western Oklahoma State College and Southwestern Oklahoma State University and earned her Doctor of Physical Therapy degree from the University of Oklahoma Health Sciences Center.
She enjoys working in a rural community and having the ability to treat a variety of age ranges and conditions. Her favorite aspect of physical therapy is being able to cheer on and assist people in reaching their goals.
In her free time, she enjoys going to concerts, watching football, and spending time with her husband, dog, family, and friends.

Clinic DirectorCaleb was raised in Pauls Valley and graduated high school in 2014. During his time at PVHS, Caleb was active in football, baseball, and the band. After high school, he attended college at East Central University where he majored in Athletic Training. Caleb was the Athletic Training Student of the year his junior and senior year. He completed his Bachelor’s degree and earned his athletic training certification in 2018. Caleb received his Doctor of Physical Therapy from South College located in Knoxville, Tennessee. Caleb lives in Shawnee with his wife Amanda and their two dogs, Weasley and Spud Lee. When Caleb is not in the clinic, he enjoys video games, working out, and spending time with family and friends.

Physical Therapist AssistantEmilea was born and raised in Tishomingo, Oklahoma. Growing up, she participated in various sports and activities including softball, track, and FFA. After high school, she earned an Associates in Arts and Science from Murray State College. She then went on to East Central University, where she obtained a Bachelor of Science degree in Kinesiology. She completed the Physical Therapist Assistant program at Murray State College. While completing the program, she spent time with PTC Davis for a clinical rotation, where she realized she would love to work for the company as a future PTA.
When not at work, Emilea spends her time outdoors, with family and friends, and playing golf. She is also an active member of OPTA and APTA. She’s excited to grow professionally and personally while at Physical Therapy Central.

Casey was born and raised in the mountains of Southeastern Oklahoma. He attended the small high school of Buffalo Valley where he enjoyed success in baseball, basketball and competitive speech and debate among other activities.

Casey attended two years of junior college while playing baseball. He then transferred to the University of Oklahoma where he completed a Bachelor of Science in Health and Sports Science in 2005. He earned a Doctorate of Physical Therapy from Langston University in 2009 where he received the Clinical Education Excellence Award. The award is presented to the student with the highest rating in the clinical education.
Casey brings this clinical excellence to Physical Therapy Central where he enjoys orthopedics, sports physical therapy, spinal dysfunction, manual therapy and sports conditioning.
To further excel his knowledge in physical therapy, Casey completed the Orthopedic Manual Therapy Residency with Evidence in motion in 2012 and became a Board Certified Orthopedic Clinical Specialist (OCS) through the APTA. He is currently pursuing a certification as a Strength and Conditioning Specialist (CSCS) through the National Strength and Conditioning Association. He has been an active member of the APTA/OPTA and an orthopedic and sports section member since 2006. Casey has been a National Strength and Conditioning Association Certified Personal Trainer since 2005.
When not at work, Casey enjoys spending time with his wife, Heather, sons Briar and Bodee, and his little girl, Kinley. He also enjoys cheering on the Sooners, cycling, snowboarding, water sports, working with his horses, hunting and fishing.If it has to do with the outdoors, he’s game!

Whitney was born and raised in Moore, OK. A graduate of Moore High School, Whitney was active in varsity soccer and student council. She also graduated in the top 5% of her class.
Whitney gained an interest in physical therapy after injuring her knee playing soccer at a young age. She went on to earn her Physical Therapist Assistant degree from Murray State College in 2014 graduating with honors and was voted her PTA class president. Before graduating from MSC, she earned her Bachelor of Science from Southern Nazarene University in 2012, majoring in Kinesiology. While attending SNU, Whitney was a starting defender for the women’s soccer team as well as team captain for her junior and senior years.
Whitney married her high school sweetheart, Matt, in 2011 and they currently reside in Moore. In her spare time, Whitney enjoys spending time with her husband, reading, playing sports, traveling, and serving in the youth group at her church.
Whitney is an active member of the Oklahoma Physical Therapy Association and American Physical Therapy Association and is currently seeking to become a Certified Strength and Conditioning Specialist.
