In order to provide the best medical care possible, it is important that we have all the tools necessary. We ask that you bring the following information to your appointment so that we can appropriately and treat your condition.

• List of previous surgeries, injuries, conditions, or other health problems
• Insurance card
• Picture identification card (i.e., driver's license, etc.)
• Referral from your physician
• Current medication(s) list (both prescription and non-prescription)

Bring comfortable clothing that you can easily move in and a pair of tennis shoes.

The initial evaluation and treatment session will usually last about 1 1/2 to 2 hours.
